Requirements of Counsel RE: Vaccination & Testing Policy - General Order 2021-13

As previously announced via an August 20 emailed memorandum to all counsel, the Court has adopted a vaccination and testing policy applicable to counsel admitted to practice before this Court and employees of their offices who will appear in person in our Courthouses and Court facilities.  (For more detailed information, please see General Order 2021-13.)

To assist in the implementation of and compliance with this policy, all admitted counsel and employees of their offices entering our Courthouses or Court facilities should bring with them:

1) a copy of their attestation form,

2) if vaccinated, proof of their vaccination,

3) if not vaccinated, a copy of their negative COVID test result taken no more than 72 hours before entering the Courthouse, and

4) if granted an exemption, proof of such exemption containing relevant terms of the exemption. 

Please be prepared to provide this information, including the date/time of your form submission email, to the judge and/or designated courtroom deputy if so requested.

A fillable version of the attestation form is available on the Court’s public-facing website under Forms/Attorney Forms and should be emailed to: vaed_counsel_attestation@vaed.uscourts.gov.
